Every player must fill out a medical release, player contract, parent contract, player contact form, and rules and guidelines form in order to play. Each player must also register with the ULA and US Lacrosse. The forms and links to register can be found on the side of this website. ULA late feels go into effect on February 17, so make sure to sign up before then! If money is an issue for any player, please contact me.
